I know lots of people who were non AOA (some top schools don’t even have AOA) and <250 step 1 who matched at BWH/MGH, especially in internal med since it’s a less competitive specialty. All the rest. There's going to be so much variability past the top 10 and even that is not quite set in stone. The top 4 are somewhat clear cut in terms of how they are viewed for fellowships, but otherwise it's not that important between top 5-20.
